Estudio de la influencia de la actividad física en la modificación de los factores de riesgo de enfermedad cardiovascular en individuos con hipercolesterolemia familiar

Abstract
La hipercolesterolemia familiar (HF) es una enfermedad hereditaria autosómica dominante que
cursa con niveles elevados de colesterol en sangre desde el nacimiento y cuya prevalencia oscila
entre 1/300 a 1/500. La exposición continua a niveles elevados de colesterol total y cLDL hace que
en estos pacientes se acelere el proceso aterosclerótico. Ello se traduce en una manifestación
temprana de enfermedad cardiovascular (ECV) y enfermedad coronaria (ECC), de forma que más
del 50% de los afectados presenta algún suceso cardiovascular antes de los 55 años. Sin embargo,
su diagnóstico suele ser tardío y su tratamiento médico insuficiente.
Objetivos.- Identificar si un nivel determinado de actividad física se asocia con una distinta
prevalencia de ECV y ECC, o de los factores de riesgo cardiovascular (FRCV) en una cohorte de
familias con HF de todo el territorio español.
Material y métodos.- Estudio transversal de la cohorte correspondiente al Registro Español de HF a
abril de 2013, de la que se recogieron antecedentes personales, FRCV, tratamiento hipolipemiante,
datos antropométricos y de presión sanguínea, niveles lipídicos, la existencia de mutación en los
genes LDLR y ApoB, el hábito tabáquico, y el nivel de actividad física (AF) mediante el International
Physical Activity Questionnaire).
Resultados.- La cohorte estaba formada por 3.918 individuos (46,8% hombres y 53,2% mujeres),
con una media de edad de 42 años y de los que 732 eran casos índice. El 65% presentaban
mutación en el gen RLDL y un 1,79% en el ApoB. El 9,9% presentaron ECVA y el 8,6% ECC, siendo
prematura en el 72,1% de los casos. La mayoría de individuos de la cohorte realizaban un nivel
moderado de actividad física (58%), seguido de un nivel bajo (30%) y alto (12%); el consumo
energético total medio asociado a la actividad física de un individuo era de 1.484 MET-
min/semana. En el análisis bivariante se observó que los individuos con un nivel alto de AF fueron
los que presentaron una menor prevalencia de ECVA (p=0,000) y de ECC (p=0,000), así como una
media de edad del primer suceso más temprana (p=0,040 y p=0,042 para ECVA y ECC,
respectivamente), aproximadamente 5 años antes. El nivel de AF también se asoció a una diferente
prevalencia de FRCV (sobrepeso/obesidad e hipertensión arterial), así como a diferentes niveles de
cHDL (p=0,009) y apo A-I (p=0,000). El modelo de regresión logística, en función del nivel de AF
realizado por los sujetos, para el riesgo de ECVA (R 2 = 41,32%) identificó como factores de riesgo:
ser hombre, tener sobrepeso/obesidad, hipertensión arterial o diabetes, el fumar, el tratamiento
hipolipemiante, los niveles de CT, cLDL y de triglicéridos y el realizar un nivel de AF alto. El modelo
para la ECC presentó una bondad de ajuste menor (R 2 = 34,0%), identificando los mismos factores
de riesgo, excepto la diabetes.
Conclusiones.- El nivel de AF se asocia con una diferente prevalencia de ECVA/ECC y con distintos
valores en los FRCV; sin embargo los hallazgos no permiten identificar una relación dosis-
respuesta, debiéndose utilizar sistemas de medición de la AF más precisos y realizar un
seguimiento a los individuos para poder esclarecer dicha relación.
