Análisis de la inhibición muscular artrogénica del cuádriceps posligamentoplastia del ligamento cruzado anterior de rodilla y su repercusión en la función del aparato extensor

Abstract
Tras la reconstrucción quirúrgica del ligamento cruzado anterior (LCA) de la rodilla,
una de las principales disfunciones es la inhibición muscular artrogénica (IMA)
cuadricipital identificada como la pérdida de su capacidad de contracción voluntaria
(parcial o total) sin lesión estructural o nerviosa directa. La IMA contribuye de forma
significativa a la amiotrofia cuadricipital, dificultando la estabilidad dinámica de la
rodilla, aumentando el riesgo de una recidiva y, si persiste, provocando mayor
degeneración articular.
La idea de este proyecto de tesis doctoral surge de la influencia clínica de la IMA en la
función del aparato extensor de la rodilla y la recuperación muscular observada tras la
reconstrucción del LCA. Así se pretendió analizar varios aspectos como son el
ascenso activo de la rótula posquirúrgico, la correlación de factores predisponentes de
la IMA, como el derrame sinovial y el dolor, la repercusión específica sobre cada grupo
muscular del cuádriceps –vasto medial (VM) vs. lateral (VL)–, el nivel de afectación
según el nivel de actividad y la capacidad funcional previa a la lesión y si la
dominancia juega algún papel.
Para ello se diseñó un estudio de cohortes con una muestra de 61 individuos, de 18 a
65 años, con previa reconstrucción quirúrgica del LCA con injerto autólogo de
isquiotibiales (semitendinoso y recto interno), con o sin meniscectomía asociada. Se
valoraron ambas extremidades a 0º y 30º pre y posintervención quirúrgica (al 4º día,
1er, 3er y 6º mes) midiendo las variaciones de la distancia radiológica desde polo
inferior de rótula a tuberosidad tibial anterior (DTTAR), en reposo y en contracción; la
fuerza máxima voluntaria cuadricipital, VM y VL con dinamometría y acoplamiento con
electromiografía de superficie; y la perimetría central rotuliana y a 5, 10 y 15 cm del
polo superior de rótula, así como el dolor mediante la escala visual analógica.
Los resultados constataron un menor ascenso activo de la rótula y un aumento del
derrame sinovial en la extremidad afecta posintervención quirúrgica que se normalizó
posteriormente en el proceso de recuperación funcional. Asimismo se observó un
aumento del dolor en reposo y una variación de los registros electromiográficos. No se
encontraron variaciones en la extremidad contralateral no afecta. La perimetría central
rotuliana y a 5 cm, junto con el registro del dolor medido con la escala de EVA,
mostraron una correlación negativa moderada con la fuerza muscular máxima
voluntaria cuadricipital a 0º y 30º. Esta correlación se invirtió y tendió a normalizarse
en el transcurso del proceso de RHB.
La repercusión de la IMA sobre los vastos cuadricipitales no fue simétrica. Se
observaron valores menores del VM y una menor contribución relativa sobre el registro
conjunto de ambos vastos en las valoraciones musculares, probablemente en relación
directa con el derrame sinovial y el dolor. No se observaron repercusiones musculares
significativas sobre la extremidad contralateral no afecta.
Así mismo se constató una relación independiente entre el desarrollo de IMA
posquirúrgica y el nivel de actividad y capacidad funcional (Score de Tegner-Lysholm),
así como una falta de correlación entre la lateralidad dominante en la extremidad
inferior y su coincidencia o no con la extremidad intervenida. Por último, se objetivó
una asimetría prequirúrgica de la extremidad afecta respecto a la contralateral,
independientemente de su dominancia, que todavía no era equiparable con la
extremidad no afecta en los registros posquirúrgicos.
