Cambios hemodinámicos en el paciente crítico antes, durante y posteriormente a la higiene y/o movilización.

Abstract

Al paciente ingresado en la UCI se le proporcionan diferentes actividades de enfermería con la finalidad de cubrir de forma global todas sus necesidades. La higiene del paciente es una intervención básica que tiene como objetivo proporcionar bienestar y comodidad, a la vez que constituye una importante medida de lucha contra infecciones, no obstante, no implica que su realización esté exenta de riesgos para el estado hemodinámico del paciente. Existen pocas investigaciones sobre la frecuencia de la aparición de efectos adversos, así como de la importancia que estos pueden tener para el paciente. La falta de datos sobre este tema hace que exista una gran necesidad de emprender investigaciones para instaurar protocolos de higiene basados en la evidencia científica para los cuidados enfermeros básicos, así como evaluar el comportamiento de las variables hemodinámicas antes, durante y después del aseo de los pacientes de la Objetivos: Saber cuáles son los cambios hemodinámicos del paciente crítico al realizar la higiene corporal y en qué frecuencia y periodo de tiempo se dan, además si es necesario el cambio de dosis de fármacos vasoactivos o de sedación. Metodología: Se consultaron las bases de datos “Pubmed”, “Web of Knowledge”, “Google Academics” y páginas web relevantes a nivel estatal y mundial. Tras aplicar los criterios de selección, se leyeron 22 artículos al competo, 11 protocolos e información obtenida a través de libros y páginas web de prestigio. Además se elaboró un protocolo base para la realización de la higiene y una tabla para anotar todos los ítems para valorar. Resultados: Los resultados que se esperan basados en la bibliografía científica, sería registrar un elevado porcentaje de eventos adversos durante la realización de la higiene o movilización del paciente, pero la mayoría de estos eventos desaparecerán pasados 15-30 minutos de la técnica, sin suponer un riesgo al paciente. Conclusiones: A consecuencia de los resultados que se esperan, creemos necesario el desarrollo y la implantación de un protocolo de enfermería para la higiene de los pacientes críticos, que esté basado en un mayor conocimiento de las complicaciones o efectos adversos que aparecen durante la realización de la técnica, y desarrollar una “base” para el criterio enfermero en el momento de la decisión sobre realizar la higiene.

Different nursing activities are developed to patients on ICU in order to cover all their needs. Patient’s Hygienic is a basic nursing intervention which aim is to provide well-being and comfort. Furthermore it constitutes an important measure to avoid infections. However, this doesn’t mean that the realization of this procedure is risky free for the hemodynamic patient state. It exist a few investigations about the frequency of the adverse effects appearance during the hygienic technics among patients on ICU as well as the importance that these can have to the patient. The lack of data about this theme makes necessary to do investigation in order to establish some hygiene protocols based on the scientific evidence for basic care nursing activities as well as to evaluate the behaviour of the hemodynamic variable before, during and after the ICU patient hygienic. Aim: To know which are the hemodynamic changes of the patient during the body hygienic and in which frequency and period of time are they usually done. As well as reporting the necessity of changing the sedation and vasoactive drugs dose during this Methodology: In order to develop this investigation Project, I consulted some database like “Pubmed”, “Web of Knowledge”, and “Google Academics”. After using all this selection criteria, I red 22 articles, 11 protocols and information from other prestigious books and web pages. In order to make this project I made a basic protocol about how to do a body hygienic and a table to make a note of all the valuable information. Results: The expected results, due to the studies based on the scientific bibliography, were a high percentage of adverse reactions during the body hygiene to the patients on ICU either their mobilization. However, all this events where expected to disappear 15-30’ minutes after the technic had been done, without involving a high risk to the patient. Conclusion: It is thought necessary to develop and implement a nursing protocol for the body hygienic for all the patients on ICU, which has to be based in a better knowledge of all the complexity or the side effect appeared during the realization of this technic. And develop a “base” for the nursing criteria in the moment of the decision about whether or not to do the hygiene.
