Potencial cariogénico de las bebidas de origen vegetal en comparación a la leche de origen bovino. Revisión bibliográfica

Abstract
Introducción: el consumo de bebidas de origen vegetal como sustitutos de la leche de origen animal no sólo va en aumento en la población adulta, sino que también en la pediátrica. El efecto de este tipo de bebidas ha sido ampliamente estudiado a nivel nutricional, pero no a nivel de acidez, caries o salud oral en general. Objetivo: el objetivo de esta revisión bibliográfica fue evaluar el efecto de las bebidas de origen vegetal sobre la salud oral del paciente pediátrico. Material y métodos: se llevó a cabo una búsqueda bibliográfica en la base de datos PubMed en la que se consultaron los artículos que cumplían con nuestros criterios de inclusión: publicados en los últimos 5 años, en inglés o castellano, que tratasen sobre el potencial cariogénico de leche bovina y/o sustitutos, revisiones bibliográficas y estudios clínicos. Resultados: tras aplicar los criterios de inclusión y exclusión se seleccionaron 6 artículos considerados como relevantes para llevar a cabo esta revisión bibliográfica, a los que se sumaron 5 artículos con fecha de publicación anterior a nuestros criterios de inclusión por su relevancia. Se seleccionaron estudios clínicos, estudios in vitro y revisiones bibliográficas. Conclusiones: las bebidas de origen vegetal más estudiadas son la de soja y la de almendra. En dichos estudios se confirma que son más cariogénicas que la leche de origen bovino debido no solo a la presencia de azúcares libres, sino también a su acidez. No obstante, sigue existiendo un vacío en la literatura en relación a la bebida de origen vegetal menos cariogénica puesto que no se encuentran estudios que analicen el potencial cariogénico del resto de alternativas existentes a la leche de origen bovino como puedan ser la bebida de arroz o quinoa.
Introduction: the consumption of plant-based beverages as a substitute of animal milk is not only increasing among the adult population but also among the pediatric population. The effect of these types of beverages has been widely studied at a nutritional level but not with regard to acidity, caries or oral health in general. Objective: the aim of this review of the literature was to evaluate the effect of plant based beverages on the oral health of pediatric patients. Material and methods: a search of the literature was carried out using the PubMed database and the articles that met our inclusion criteria were consulted. These articles had been published in the last five years in English or Spanish, they were on the cariogenic potential of bovine milk and/or substitutes, and were literature reviews and clinical studies. Results: after applying the inclusion and exclusion criteria, six articles that were considered relevant for this literature review were chosen together with another five articles with a publication date that was before that in our inclusion criteria given their relevance. Clinical studies, in vitro studies and literature reviews were selected. Conclusions: the plant-based beverages that have been studied the most are soybean and almond milk. The studies confirm that these drinks are more cariogenic than bovine milk due not only to the presence of added sugars, but also acidity. Nevertheless, there is still a vacuum in the literature with regard to the least cariogenic plant-based beverage given that there are no studies that analyze the cariogenic potential of the other alternatives to bovine milk that exist such as rice or quinoa beverages.
